After noticing replied tickets were being lost, amongst other issue, we are going to have to shut down OS ticket as there appear to be far too many bugs to be used on a production server. I hope one day they could be resolved.
What version are you running? Have you consider it a wrong implementation? are you tickets being lost upon a fresh install?
I particular have been using ostickets since 2009 without losing tickets.

I found the problem. I had caching turned through a varnish instance on the server. Turned of caching and it solved all my issues.
