Hi I have installed a fresh OST v1.10 on Debian Jessie, PHP v7.0.14, Apache v2.4.10 and if I want to create a new ticket, there is no Contact Information forms and if I press on Create Ticket, a new ticket will be created with confirmation "Thank you for contacting us. A support ticket request has been created and a representative will be getting back to you shortly if necessary." but without entering any username, email...
How can I debug this error?
