(This version is working on my installation)
This is a FAST collection of the process to do the following:
Add a database populated drop down box to tickets
add ui controls for feature to the admin panel
I will try to pretty this up a little later but do not have the time to do so now.

If you have followed these instructions prior to 23 Apr 2013, there were a couple small changes missing from the instructions. I've updated the instructions and will include the changes here:

Since email doesn't have a "agency" value we're going to hard code one for now. You may want to create a default value in the database of "Emailed" and change this number to that.

at line 382 add:

The database table changed from 1.6ST to 1.7ST. The ost_agencies table should have an additional column called 'notes'. If you were using this with 1.6ST and are upgrading or followed the instructions prior to the date listed above you should run the following MySQL query to add the additional column.
ALTER TABLE `ost_agencies` ADD `notes` VARCHAR(255) NOT NULL AFTER `upDate`;
