1.9.12 : manage custom form

Hi,

my version is 1.9.12 OST
Is it possible to create custom form dedicated to specific department, with mandatory sequential field "eg checlbox"for my agents.
Each new intervention in the ticket by a agent must comply neccessite a workflow by checking the next box in the list. am I clear?

    What you are describing sounds more like the tasks feature coming in the next version.
