[resolved] Issue with new ticket empty form

Hello

I have install and configure the OS  ticket support on support.kwixx.fr (see screenshot : https://drive.google.com/file/d/0B3gA2073oCTBZnlRcDVqSXViMUk/view?usp=drivesdk )

And the form is totally empty, i dunno what to do or what is the issue.

Could someone help me ?
