How to use custom forms??

i have created custom form but when i acces to th ticket i can´t see nothing.
please, somebody could help me?

  • Ensure that the fields are enabled at: Admin panel -> Manage -> Forms.
    Click on the config button for each item, click on the settings tab and ensure that the proper check boxes are checked for your use case.
