Cannot submit/open a new ticket

When I want to create new ticket and submit "Open", the screen will display white page (Please find the attached 01.png file for your reference).

    A white page indicates a PHP error.  Please check your logs.

    I cannot tell from your image what it is... If you are getting a white bar then you likely have an AJAX issue.

    No one here can assist you with out more information.  Please see link above.

  • Before the issue, I just added a new help topic and create a new ticket form.

    Have tried to delete the new ticket form / all custom ticket form, but the issue is as same. 
    I can provide the related log to checking,  please let me know which log file is need?
