CAPTCHA Text does not appear on refresh

When Open New Ticket is launched in a new browser, the Captcha text will appear with no problem. But when the page is refreshed, or navigated from other links (eg from menu bar or ticket status), the Captcha text refuses to appear.

Does anyone know why and how to resolve this, please? Many thanks in advance.
